New Patient Registration

We only accept patients who reside within the practice boundary. If you wish to check whether you can register with the practice please contact us on 01224 208312.

When registering, to reduce the amount of paper handling, it is preferred if you can download the NHS registration form and New Patient Questionnaire, complete and email to the practice at  Please note once your documents has been received, the registration process takes approx. 48hrs.  To check on the status of your registration after this time please contact Reception on 01224 208312 as no confirmation will be sent.

For those patients who do not have computer access,  you can complete forms available from the practice - Please bring your yellow NHS Medical card, if you have one, and those of any other persons registering with you. 

**please note we require a completed copy of the NHS registration Form & New Patient Questionnaire for every new registration**

For information - the new patient questionnaire, allows us to provide medical care in the interim period, while your medical records are transferred from your old practice to this one.

All new patients will be offered a registration appointment with the Practice Nurse or a GP.

New patients are registered from within practice boundaries and if you move out of the practice area, you will need to register with another practice as soon as you move. We can provide information about how to find a new practice. Please ensure that the practice has your correct name and address.

Facilities and Access

Disabled Access

Our Health Centre building is fully accessible to disabled patients.

A lift facility exists between the upper and lower floors and all the patient areas including waiting room, consulting rooms and toilets have wheelchair access. Designated disabled parking spaces are located in the car park for the Health Centre, there is also a Disabled Parking /Drop off point just outside the entrance to the Health Centre.

Car Parking

There is a car park to the rear of the Health Centre for short-term patient parking. The barrier will rise when you enter the car park.

To exit the car park please ensure you collect a barrier exit number from Reception.

Bus Service

A regular bus service operates to the site via First Bus Red Lines 1 & 2.
